Is medical marijuana allowed in Islam?

It depends. Cannabis that makes you high is an intoxicant, and most scholars say it is forbidden. Some scholars allow a carefully prescribed medical form for a serious condition when nothing else works, under the necessity rule. CBD without THC is viewed more leniently. Using it for recreation stays forbidden, even where it is legal.

Why cannabis is treated as forbidden

Islam forbids intoxicants, not just alcohol. The Quran tells believers to shun them (Quran 5:90), and the Prophet ﷺ said: "Every intoxicant is Khamr and every intoxicant is forbidden" (Sahih Muslim 2003). Scholars apply this to cannabis because THC, its active ingredient, alters the mind.

When a man told the Prophet ﷺ that he made alcohol as a medicine, he replied: "It is no medicine, but an ailment" (Sahih Muslim 1984). Many scholars use this to say that intoxicants shouldn't be used for treatment when other options exist.

When some scholars allow it

The Quran gives a general rule for real emergencies: forbidden things become allowed for someone compelled by necessity, who is not seeking pleasure or going beyond what is needed (Quran 2:173). Some contemporary scholars apply this to prescribed medical cannabis, with strict conditions:

  • The condition is serious, such as severe chronic pain, certain forms of epilepsy or nausea from cancer treatment.
  • Trusted, qualified doctors say there is no equally effective alternative.
  • It is prescribed and supervised, in the smallest effective dose.
  • A non-intoxicating form is chosen where one works, such as CBD or a licensed pharmaceutical product.
  • It is taken as medicine, not for the high, and not smoked for enjoyment.

Other scholars hold that cannabis that intoxicates is never acceptable as medicine. Because the views differ and your health is involved, speak to a knowledgeable imam about your case and follow your doctor's advice. For one example in detail, see Can I use medical cannabis for anxiety as a Muslim?

Prayer and daily life

A person who is intoxicated is not allowed to pray in that state:

يَـٰٓأَيُّهَا ٱلَّذِينَ ءَامَنُوا۟ لَا تَقْرَبُوا۟ ٱلصَّلَوٰةَ وَأَنتُمْ سُكَـٰرَىٰ حَتَّىٰ تَعْلَمُوا۟ مَا تَقُولُونَ "O believers! Do not approach prayer while intoxicated until you are aware of what you say" Quran 4:43

If a prescription makes you drowsy or impaired, ask your doctor if the timing of doses can be arranged around the prayer times. Prayer remains a duty even when you are ill, and you may pray sitting or lying down if needed. Never skip prayers because of how a treatment makes you feel without asking for advice first.

Legal does not mean halal

  • Legality varies. In many US states and in some countries, cannabis is legal, but this does not change the Islamic ruling for recreational use.
  • Dispensary products are not the same as medicines prescribed for a diagnosed illness.
  • Be honest with your doctor about your faith, so they can offer options.
  • If you struggle with dependence, there is help available, and asking for it is a strength.

Key points
  • Cannabis that causes intoxication is forbidden for recreation.
  • Some scholars allow prescribed medical use for serious conditions under strict necessity.
  • CBD without THC is generally viewed more leniently.
  • Never pray while impaired, and ask about timing your doses.
Tip: Before starting any cannabis-based treatment, ask your doctor: "Is there a non-intoxicating option I can try first?" That question often opens up better choices.
