Is scalp micropigmentation haram?

It depends, and many scholars advise against it. Scalp micropigmentation (SMP) places pigment under the skin with needles to look like short hair or a fuller hairline. Because that is how a tattoo works, many scholars place it under the tattoo prohibition. Some allow it as treatment for medical hair loss, such as scarring or alopecia.

Why it looks like a tattoo

The Prophet ﷺ said that Allah has cursed the one "who tattoos (herself or someone else) or gets herself tattooed" (Sahih al-Bukhari 5937). Scholars describe tattooing as pricking the skin and filling it with pigment so that it stays. SMP does exactly that on the scalp. It is sold as "semi-permanent" because the pigment is meant to last for years, though it may fade over time.

So the cautious and most common view is that SMP counts as a tattoo, even though the result looks like stubble and not like a picture. Because of that, many scholars advise against it for ordinary baldness.

Why some scholars allow it

Others point out that the tattoo prohibition is about changing the body for beauty, while SMP can be treatment. They compare it to the Companion Arfajah, who lost his nose in battle and was told to get a gold one (Sunan Abi Dawud 4232). In their view, covering scars or the effects of a medical condition restores something lost and is a need, not vanity. Those scholars usually say ordinary male-pattern baldness is a weaker case, since there are other ways to deal with it.

If you have a medical reason, such as scarring or a condition like alopecia, ask a trusted local imam and your doctor together.

Safer alternatives

  • Hair transplant: uses your own hair, and most contemporary scholars allow it.
  • Shaving your head: a clean, bald look that is allowed at any time.
  • Hairpieces: allowed by many scholars for real baldness, with conditions.
  • Hair fibers or sprays that wash out: temporary and not tattooing. Wash them off before ghusl so water reaches the scalp.

If you've already had it

If you had SMP before you became Muslim, don't worry. Your sins are wiped clean when you accept Islam, and the pigment sits under the skin, so it doesn't affect wudu or ghusl. See Can I make wudu with a tattoo?

If you had it after becoming Muslim, repent and move on. There is no need to panic, and scholars generally don't ask you to undergo painful treatment to undo it. Removal is a personal choice.

Key points
  • SMP uses needles and pigment, so many scholars treat it as a tattoo.
  • Some scholars allow it as treatment for scarring or medical hair loss.
  • For ordinary baldness, safer alternatives exist.
  • An old SMP doesn't affect wudu, ghusl or prayer.
Tip: If you're tempted, first ask a clinic exactly what is used and how long it lasts, then talk to your imam. Don't decide on a permanent mark in the same week you feel upset about hair loss.
