Yes, in most cases you should tell your future spouse about a significant mental illness before the nikah. Islam values honesty in marriage, and a condition that affects daily life is something a partner has a right to know. You don't have to share every detail on the first meeting, but they should know before they commit.

Honesty is the foundation of a nikah

Marriage in Islam is a contract, and contracts depend on both sides knowing what they agree to. The Prophet ﷺ said that when two people in a deal are truthful and make things clear, their deal is blessed, and when they hide and lie, the blessing is wiped out (Sahih al-Bukhari 2079). Allah also says:

يَـٰٓأَيُّهَا ٱلَّذِينَ ءَامَنُوا۟ ٱتَّقُوا۟ ٱللَّهَ وَقُولُوا۟ قَوْلًا سَدِيدًا "O you who have believed, fear Allah and speak words of appropriate justice." Quran 33:70

Classical scholars of all four schools discussed serious health conditions that were hidden before marriage. They differed on which ones count, but many held that hiding a major condition could give the other spouse the right to end the marriage. Being open from the start protects you both.

Past sins vs. health conditions

These are not the same. Past sins that Allah has covered should usually stay covered. See Do I have to tell my future spouse about my past? Mental illness is not a sin and nothing to be ashamed of. It is a health matter, like diabetes or asthma, and it may affect finances, children, medication or daily routines. See Is depression a sign of weak faith in Islam?

What and when to share

  • Timing: once things are getting serious, before an engagement or nikah, not on the first meeting.
  • What matters: the diagnosis in simple words, how it affects you, and how you manage it (treatment, medication, support).
  • What's optional: a mild, resolved episode years ago may not need detailed discussion. Use good judgment and ask a trusted scholar if unsure.
  • Involve the right people: a wali or trusted family member can help keep the conversation calm and private.

Framing it with confidence

Share it as part of who you are, not a confession. Something like: "I have bipolar disorder. I'm stable on treatment, I see my doctor regularly, and here's what support helps me." A good spouse will appreciate the honesty, and someone who rejects you for it was not the right match.
